For the above insurance carriers patients will be responsible for any applicable co-payments, co-insurance or deductibles.
We will bill the patient for any carrier whom we are not contracted with (as required by federal law). In the event of a financial hardship, we will consider Charity Care. In this case, the patient will be asked to provide proof of financial hardship (as required by federal law).
