Please use the following forms to send requests to the Library. The services are for the Hospital staff and our affiliated personnel only. If you have questions or need assistance, please call us at (732) 745-8545.

• Literature Search Form 
  Use this form to request computer literature searches for a list of articles on your topic. For example, if you would like us to search for the latest information on pain management, this is the form to use.
  
  
•  Article Request Form 
  Use this form to request articles the Library does not have. We will get the articles you need from other libraries for you. It can take several days to a couple of weeks for the articles to come in, depending on where we get them. If you need the articles urgently for patient care, please call us instead. In some instances, there may be a charge for the articles obtained. If you are an attending physician or senior administrative staff member, you may use this form to request copies of articles the Library owns.
  
  
• Book Request Form 
  Use this form to request books the Library does NOT own. We will borrow the books from other libraries for you. For a book the Library does not own but you feel we should, please e-mail us at mlibrary@saintpetersuh.com.
