Geriatrics
A specialized team of physicians, nurses, social workers and other healthcare professionals works collaboratively in the Geriatric Evaluation and Management Services (GEMS) program to serve patients with multiple medical conditions, Alzheimer's disease, and other forms of dementia. GEMS includes a medical and surgical history, a review of the patient's medications, physical and cognitive examinations, and a screening for depression. Support Services
Along with primary care physicians and specialists, Saint Peter’s Comprehensive Care Group at Monroe provides a full range of support services. If the need ever arises, we have a social worker to help coordinate care with outside agencies such as a nursing home or long-term care facility. In addition, we can help with financial needs like prescription assistance or insurance programs.
