Through internal medicine and primary care, staff of the Comprehensive Care Group at Piscataway provide an integrated program of care for older adults. Additionally, a specialized team of physicians, nurses, social workers and other healthcare professionals works collaboratively in the GEMS (Geriatric Evaluation and Management Services) program to serve patients with multiple medical conditions, Alzheimer's disease, and other forms of dementia. GEMS includes a medical and surgical history, a review of the patient's medications, physical and cognitive examinations, and a screening for depression. The team then counsels the patient and family and develops a personalized plan of care.
